Sign Up Information

In order to participate student athletes will need the following:

1. Fill Out the Online registration form using their school email address (This will be active October 6 - 14, 2025)
2. Student Athletes must have an up to date physical on file with the school nurse
3. Student Athletes must take an impact test, good for 2 years.
4. A Nurse Interview

Impact Tests will be given during schools hours November 3 - 7, 2025 in the athletic office.  Please have your student athlete stop down and make an appointment. 

 

All consent forms are due to the Health Office by Thursday, October 16, 2025

* In order to remain eligible for participation in the district's interscholastic athletic program, student athletes must maintain a minimum academic average of 72.00% for the previous marking period *
