Sign Up Information

In order to participate student athletes will need the following:

1. Fill Out the Online registration form using their school email address (This will be active October 7 -15, 2024)
2. Print and turn in their consent form to the school nurse
3. Student Athletes must have an up to date physical on file with the school nurse
4. Student Athletes must take an impact test 
5. A Nurse Interview

* In order to remain eligible for participation in the district's inter-scholastic athletic program, student athletes must maintain a minimum academic average of 72.00% for the previous marking period *
